OSHA requests self-investigation... Co. cites the cause as "stupid employee"

At the recent "OSHA stand down meeting on Construction Falls" at the University of Texas - Arlington, we received a heads-up on how not to respond when OSHA asks your company to self-investigate an amputation injury.

Jack Rector, the OSHA Director in the Fort Worth office, says he's amazed how
many companies send responses that basically say "our employee did a stupid thing", with no significant actions identified to remedy an unsafe situation. He put us on warning that his office will try to visit all companies that send responses that do not include a detailed plan on what the company is doing to prevent similar accidents in the future.
